POSITION SUMMARY:

Health Services Director (LPN)-- Responsible for providing direct resident care and supervision of residents needs per individualized service/care plan, monitors residents for change of condition and signs/symptoms of illness according to federal and state laws and regulations and according to the policies of the Community. Responsible to communicate and document resident changes or concerns to Health Services Director. Assist in supervision of resident assistants.

REPORTS TO: EXECUTIVE DIRECTOR

POSITION RESPONSIBILITIES:

  1. Complies with all federal and state laws, including state Resident Rights’ law.
  2. Complies with all Community policies and procedures
  3. Knowledgeable of state regulations pertaining to resident care.
  4. Monitors resident health status.
  5. Ensures resident privacy and dignity.
  6. Assesses, documents and reports change in resident condition.
  7. Provides emergency medical intervention as needed.
  8. Administers medications/treatments per community policies/procedures.
  9. Ensures implementation resident’s individualized care plan.
  10. Provides recommendation for changes to resident care plan.
  11. Communicates residents care plan changes to residents, families and resident assistants.
  12. Documents resident medical data per community policy.
  13. Documents new physician orders and communicates change to nursing staff.
  14. Monitors ADL documentation per community policy.
  15. Documents, investigates and timely reports all resident incidents.
  16. Immediately intervenes, reports and investigates suspected or alleged verbal, physical, sexual abuse or theft from resident.
  17. Maintains safe storage of medication.
  18. Reviews monthly weight/vital sign documentation.
  19. Implements and monitors behavioral management interventions.
  20. Assist residents with daily ADL’s.
  21. Maintains confidentiality of residents, families, and community.
  22. Maintains a safe and clean environment for residents and staff.
  23. Practices universal precautions.
  24. Complies and oversees infection control procedures per community policy.
  25. Reports safety concerns and maintenance issues to supervisor.

EMPLOYMENT REQUIREMENTS:

  1. Willingness to adhere to assisted living principles in provided services.
  2. Ability to utilize and apply basic management principles.
  3. Ability to recognize and communicate problems effectively.
  4. Ability to relate to the public, Residents, families, staff, other professionals appropriately.
  5. Ability to communicate well, verbally and in writing.
  6. Ability to maintain accurate records.
  7. Ability to adhere to and communicate policy and procedures.
  8. Ability/willingness to improve skills of self and staff; to be active professionally
  9. Ability to lift to 50 pounds occasionally, 30 pounds regularly.
  10. Maintains appropriate and required license.

TRAINING AND EXPERIENCE:

Licensed Nurse with a minimum of at least one-year in-home health or geriatric nursing in a similar position. Rehabilitative and restorative experience helpful. CPR/First Aid certification. 

Full-time and on call during the week, rotating call schedule with other Trustwell communities on the weekends ensures work/life balance.
